29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Drill site: On or about March 3, 2015, and at times prior thereto, the employer failed to ground to earth one trailers located at the left rear corner of the rig site. This condition exposed the employees to electrical shock and burns hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Drill site: On or about March 3, 2015 and at times prior thereto, the strain reliefs from an extension cord plug and a circuit breaker box were damaged. This condition exposed the employees to electrical shock and burns hazards. a. A 50-foot 12 AWG electrical extension cord located behind the rig crew's trailer had a broken strain relief on the plug side. b. A cable going into high voltage electrical circuit breaker box located on side of Directional Driller's trailer had a broken strain relief.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Drill site: On or about March 3, 2015, and at times prior thereto, employees were working on Basin Drilling Rig #104. Behind the crew trailers were three extension cords plugged into one another (daisy-chained). Manufacture instructions specifically states do not plug one extension cord into another. This condition can exposed employees to electrical shock hazards.
