29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made: In the docking shipping and production areas, boxes containing finished product were stored on the pedestrian and forklift pathway, in that no sufficient clearance on the aisles was allowed for pedestrian and forklift traffic, exposing employees to struck by hazards.

29 CFR 1910.178(l)(2)(ii): Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. On the warehouse and production areas, forklift operator was not evaluated to operate the powered industrial truck before being placed in the workplace, exposing employees working in the docking shipping and production areas to a potential struck by hazard.
