29 CFR 1926.1404(c): The A/D director did not review the applicable assembly/disassembly procedures immediately prior to the commencement of assembly/disassembly unless the A/D director understood the procedures and had applied them to the same type and configuration of equipment (including accessories, if any).  At or near the job site located at 309 American Rd in El Dorado, Arkansas,  employees were required to disassemble a lattice boom crane under the direction of an inexperienced A/D director who did not review the disassembly procedure prior to beginning the disassembly. These conditions exposed the employees to the hazard of being struck by falling objects.

At or near the jobsite located at 309 American Rd in El Dorado, Arkansas, employees were required to disassemble a lattice boom crane, under direction of a A/D director who was not a competent or qualified person. These conditions exposed the employees to the hazard of being struck by falling objects.

29 CFR 1926.1404(f)(1): When pins (or similar devices) were being removed, employees must not be under the boom, jib, or other components, except where the requirements of paragraph (f)(2) of this section were met.  At or near the job site located at 309 American Rd in El Dorado, Arkansas, where employees were dismantling a lattice boom crane, the employees stood beneath the boom while removing the connecting pins. These conditions exposed the employees to the hazard of being struck by falling boom sections.

29 CFR 1926.1404(h)(7): The boom sections, boom suspension systems (such as gantry A-frames and jib struts), and components were not rigged or supported to maintain stability upon the removal of the pins.  At or near the job site located at 309 American Rd in El Dorado, Arkansas, employees were required to remove the pins from the connecting lugs on a lattice boom crane, without blocking or otherwise supporting the boom to prevent movement. These conditions exposed the employees to the hazard of being struck by the falling boom.

At or near the jobsite located at 309 American Rd in El Dorado, Arkansas, where employees were required to disassemble a lattice boom crane,  the operator who was in the cab, did not have the operator's manual readily available. These conditions exposed the employees to the hazard of being struck by falling objects.
