(a) Employees working in the elevator shaft were not protected from electrical shocks because the flexible extension cord used to energize the work lights did not maintain the proper electrical path to ground, because the ground pin on the cord had been removed or broken off.

The A frame ladder was folded closed and leaning against the wall, the A frame ladder is designed to be used in an opened and locked position.
