This was most recently evident during the inspection of the facility located at 2500 Old Hadar Rd & 2300 Riverside Blvd. Norfolk, NE 68701; where the employer has employees performing maintenance on equipment such as the exhaust fans located on upper work platforms where there is missing guard railing.  Instances include but are not limited to:  i.  Employees using a fixed ladder as means of access to the top of the Bema Mixer so they can look into the overhead observations door to verify the machine is not plugged up.  ii.  Employees using a fixed ladder with unprotected access point, as means of access to the catwalk along the north wall between the main production line and the dust collector room.   iii.
