29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The employer is failing to protect employees from electrical and caught between hazards associated with performing maintenance and cleaning machines with stored energy. The most recent occurrence of this was found at the facility located at 2500 Old Hadar Rd, Norfolk, NE 68701, where the employer has employees cleaning and maintaining equipment within the facility that require the implantation of lockout and/or tag out as means of preventing unintentional engergization of the equipment. Instances of include but are not limited to: i. Employee removing a piece of metal from the Bematic Opener #1 without locking out against all hazardous energy. ii. Employee assisting in the removal of a piece of metal from the Bematic Opener #1 without locking out against all hazardous energy. iii. Employee assisting in the maintenance in scheduled maintenance repairs on both Bematic Openers 1 & 2 without locking out against all hazardous energy.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: The employer is failing to train employees on the recognition of hazards associated with hazardous energy sources. The most recent occurrence of this was found at the facility located at 2500 Old Hadar Rd, Norfolk, NE 68701, the employer has employees assigned duties requiring them to perform authorized work without having had received authorized training. Instances of effected employees performing authorized employee work include but are not limited to: i. Employee performing maintenance on the Bematic Opener #1 and #2 without having been trained on the various types of hazardous energy or the use of written procedures. ii. Employee performing cleanup operations on the Bematic Openers #1 and #2 without having been trained on the various types of hazardous energy or the use of written procedures.
