29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:    a) On or about 03/17/15 at the Coiler area; employer failed to protect employees, involved in servicing and maintenance activities of the coiler, from the struck by hazards of the unexpected movement of the coiler tubs, when the Lockout/Tagout procedures for the Mill 8 Coiler did not include shutting off the air supply. 29 CFR 1910.212(a)(3)(ii): The guarding device was not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    a) On or about 03/10/15 at the Coiler area; employer failed to protect employees who operated the expansion machine from the caught between hazards at the point of operation when the side guard on the machine was not secured and easily removed.       b) On or about 03/10/15 at the Coiler area; employer failed to protect the employees who operated the expansion machine from caught between hazards when the inlet opening was 1.5 inches with a distance of 3.5 inches to the point of operation.       c) On or about 03/10/15 at the Coiler area; employer failed to protect employees who operated the flattening machine from caught between hazards when the guard on the inlet opening was 1.5 inches with a distance of 6.5 inches to the  point of operation.       d) On or about 03/17/15 at the area between Mill 1 & 2; employer failed to protect employees who operated the Dennison press from caught between hazards at the point of operation when the two palm buttons were located at a distance of 18 inches apart, did not have a guarding device and allowed one hand activation.      e) On or about 03/17/15 at the area between Mill 4 & 5; employer failed to protect employees who operated the bead machine from caught between hazards at the point of operation when the two palm buttons, at a distance of 19 inches apart, did not have a guarding device and allowed one hand activation.           ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation;     a) On or about 03/10/15 at Mill #4; employees who performed service and maintenance work on the mills, such as but not limited to, rebuilding the mill, checking shaft bearings, rebuilding shaft bearings, were exposed to caught between hazards when the employer did not provide training on the methods and means required for energy isolation prior to performing these tasks and activities.        b) On or about 01/101/5 at the Coiler for Line #8; employees who performed service and maintenance work on the coiler and puller rolls, such as but not limited to, changing over the coiler, changing the puller rolls, were exposed to caught between hazards when the employer did not provide training on the methods and means required for energy isolation prior to performing these tasks and activities.      ABATEMENT CERTIFICATION REQURIED     Markin Tubing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFPR 1910.147 (c) (7) (I) (A), which was contained in OSHA inspection number  315178293,  Citation  Number 1 Item  Number 5 and was affirmed as a final order on March 17, 2011 with respect to a workplace located at 1 Markin Lane, Wyoming, New York 14591.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    a) On or about 03/10/15 at Mill #4; employees who performed service and maintenance work on the mills, such as but not limited to, rebuilding the mill, checking shaft bearings, rebuilding shaft bearings, were exposed to caught between hazards when they switched off the mill, left the key in the selector switch and the employer did not require the application of a lock on energy isolation device for this equipment prior to these types of activities.             b) On or about 03/10/5 at the Coiler for Line #8; employees who performed service and maintenance work on the coiler and puller rolls, such as but not limited to, changing over the coiler, changing the puller rolls, were exposed to caught between hazards when they shut off the disconnect to the coiler but the employer did not require the application of a lock to the closed disconnect for the coiler prior to these types of activities.           c) On or about 03/10/15 at the expansion machine; employees who performed service and maintenance activity, such as but not limited to, changing the heads, on this equipment were exposed to caught between hazards when they disconnected the air line but the employer did not require the application of a lock to the air line prior to these types of activities.       ABATEMENT CERTIFICATION REQUIRED     Markin Tubing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFPR 1910.147 (d) (4) (i), which was contained in OSHA inspection number  315178293,  Citation  Number 1 Item  Number 11b and was affirmed as a final order on March 17, 2011 with respect to a workplace located at 1 Markin Lane, Wyoming, New York 14591.
