29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Construction site: On or about March 11, 2015, and at times prior thereto, two employees were working on attaching a steel beam grid to building A from an aerial lift without any type of fall protection. This condition exposed the employees to fall hazard greater than 6-feet.
