29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have brakes set to prevent movement: On or about 3/11/2015 employees were exposed to struck by hazards from a Toyota forklift. An employee was struck by an unoccupied rolling forklift that did not have the brake fully engaged.
