29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: A) Front of house (Driveway): Employees were exposed to fall hazards of approximately 13 feet while using an extension ladder for access on and off the roof. The extension ladder that was used did not extend at least 3 feet above the landing, on or about October 2nd, 2014. B) Middle of house (Driveway): Employees were exposed to fall hazards of approximately 13 feet while using an extension ladder for access on and off the roof.
