29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section:    a- Pump Jack Scaffold: Employees were exposed to falls of approximately 20ft to the concrete as the scaffold platform exceeded 14 inches of clearance due to the bracing of a fire escape. Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage.     a- Pump Jack Scaffold: An employee was exposed to scaffold collapse as a bridged section of pump jack scaffold section was not secured by a positive gripping mechanism so as to prevent failure or slippage.
