29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: (a) The competent person failed to properly evaluate an excavation which was deeper than 8 feet, to ensure that the proper cave-in protection was utilized. This exposed employees to engulfment hazards.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. (a) The employees were utilizing a fiberglass ladder for access and egress to the excavation. The employer failed to ensure that the ladder was long enough to ensure that the side rails were capable of extending 3 feet above the landing. This exposed employees to a fall hazard.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): (a) The employer failed to ensure that employees were utilizing cave-in protection while relocating a fire hydrant within an unprotected excavation that was approximately 9 feet deep (at the east end), 8 feet deep (at the east end), 22 feet long, 10.5 feet wide at the east end and 13 feet wide at the west end in class B soil. This exposed employees to engulfment hazards.
