29 CFR 1910.29(a)(4)(ii): All scaffold casters were not provided with a positive wheel and/or swivel lock to prevent movement.   On or about 9 March 2015, the employer did not provide a means for locking the wheels of five (5) job fabricated, manually-propelled mobile ladders during use, exposing employees to a fall hazard.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.  On or about 9 March 2015, the oxygen cylinders and acetylene cylinders were stored without a 20 foot separation or a fire resistance barrier, exposing employees working in the area to a fire or explosion hazard.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed so as to prevent moisture or water from entering and accumulating within the enclosures.   On or about 9 March 2015, water from a roof leak was dripping onto an electrical socket that was not rated for moist environments. Although it had a GFCI, it did not have barriers for preventing moisture from entering into the socket exposing employees to an electrical shock hazard.
