29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: Main Production Line at 2500 Old Hadar Road, Norfolk, NE 68702: The employer failed to establish and maintain an audiometric testing program for employees who are exposed to noise exceeding an 8-hour time-weighted average of 85 decibels, in accordance with the requirements of 29 CFR 1910.95(g)(1): An employee (a computer operator) was exposed to continuous noise of 87.9 dBa or the equivalent noise dose of 70.2% during the 448 minute sampling period on 03/19/2015. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.

Yet the employer is failing to provide employees training to recognize, evaluate and control exposure to hazardous chemicals including, but not limited to, Carbon Dioxide and Diesel fuel. Thermopod Line at 2300 Riverside Boulevard, Norfolk, NE 68702: Employees are required to perform tasks, such as, but not limited to, cleaning scissors exposing them to hazardous chemicals. Yet the employer is failing to provide employees training to recognize, evaluate and control exposure to hazardous chemicals including, but not limited to, Aero-Clen Plus Aerosol.
