29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  a) Plant, e-coat line - On or about 03/11/15 and at times prior thereto employees came in contact with chemicals including (but not limited to) Powercron Black Feed (PPG Product Code CF691B-524) and the employer did not assess the e-coat line to determine the associated hazards, and the need for personal protective equipment.    b) Plant, powder coat line - On or about 03/11/15 and at times prior thereto employees came in contact with chemicals including (but not limited to) Envirocron Powder (PPG Product Code PCT49118), and the employer did not assess the powder-coat line to determine the associated hazards, and the need for personal protective equipment.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.  a) Plant, e-coat line - On or about 03/11/15 and at times prior thereto employees came in contact with chemicals including (but not limited to) Powercron Black Feed (PPG Product Code CF691B-524) and the employer did not select and did not require the use of appropriate hand protection.     b) Plant, powder coat line - On or about 03/11/15 and at times prior thereto employees came in contact with chemicals including (but not limited to) Envirocron Powder (PPG Product Code PCT49118), and the employer did  and the employer did not select and did not require the use of appropriate hand protection. 29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) Facility - On or about 03/11/15 and at times prior thereto employees performed repairs/maintenance of machinery including (but not limited to) the equipment described in the document ACM Equipment Log (03/11/2015 11:17 AM) which is subjected to lockout/tagout. The employer did not develop, implement, and utilize individual written lockout/tagout procedures to protect workers from electrical and amputation hazards.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:  a) Plant, Shop Area - On or about 03/11/15 and at times prior thereto employees operated the Baldor bench grinder/buffer S.N. P599, 1800 rpm. The machine did not have a tongue guard installed on the left side (grinding wheel), and the gap between the wheel and the guard could not be adjusted to a maximum gap of 1/4 in.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1200(g)(8):   The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s):  Plant, e-coat line - On or about 03/11/15 and at times prior thereto employees cleaned parts with compressed air to remove coating residues and came in contact with Powercron Black Feed (PPG Product Code CF691B-524). The employer did not maintain a copy of the Safety Data Sheet (SDS) at the workplace to inform workers of the toxic/hazardous properties of the material.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
