29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) B Cell, Door 41; On or about March 3, 2015, the employer failed to report an employee's in-patient hospitalization to OSHA. The tire of a Toyota Powered Industrial Truck Model 8FBCU30 (Serial No. 63614) slipped off the dock plate onto the concrete dock while the employee was trying to drive into the back of a truck for unloading. The employee tried to readjust the PIT and ended up trapping her hand between the PIT and dock door 41 injuring her left thumb.
