The tire of a Toyota Powered Industrial Truck Model 8FBCU30 (Serial No. 63614) slipped off the dock plate onto the concrete dock while the employee was trying to drive into the back of a truck for unloading.  The employee tried to readjust the PIT and ended up trapping her hand between the PIT and dock door 41 injuring her left thumb.
