29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): Worksite located at 121st St & Delaware Ave. Tulsa, OK 74008. On or about March 11, 2015, and at times prior thereto, adequate protective systems, such as sloping, benching, or shielding were not used to protect employees while working in a trench at a depth of approximately 9 feet. The employees were exposed to engulfment or crushed-by hazards from collapsing soil.
