29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:     A)    RC Family Wood Products, LLC, located at 5590 State Route 47, Houston, OH 45333: On or about March 9, 2015, the employer failed to properly train employees to lockout/tagout the Brewco Resaw (Model # 3HB800, Serial# BW3L1163) before employees conducted activities such as, but not limited to, changing blades and cleaning the machine. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work.

Employees were exposed to injuries from unexpected start-up of the machine and energy release while energy sources such as, but not limited to, electrical and pneumatic were not controlled prior to conducting work.

29 CFR 1910.213(r)(4): Suitable guard(s) were not provided to reduce to a minimum the hazard due to the point of operation of the chop saw:     A)    RC Family Wood Products, LLC, located at 5590 State Route 47, Houston, OH 45333: On or about March 9, 2015, the employer did not ensure that adequate guarding was provided to protect employees from the rotating blade of the chop saw (green saw located in the buildings production area). The guard of the saw did not prevent employees from accessing the point of operation, exposing employees to amputation hazards while operating the saw.
