29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) RC Family Wood Products, LLC, on or about January 29, 2016, at the address of 5590 State Route 47, Houston, OH: The employer failed to ensure that energy control procedures were utilized to control potentially hazardous energy sources while an employee performed service and maintenance work on the gantry drive system of the Woodpecker Nailing Machine. The employee performing this activity in an energized condition was exposed to incoming nip points at the machines chain and sprocket drives underneath the machine.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) RC Family Wood Products, LLC on or about January 29, 2016, at the address of 5590 State Route 47, Houston, OH: The employer failed to ensure that hazardous energy sources such as, electrical and pneumatic power functions were isolated before employees performed service and maintenance work at or near chain and sprocket drives contained on the Woodpecker Nailing Machine. An employee performing this activity in an energized condition was exposed to incoming nip points underneath the machine, in that energy control isolation procedures required under 29 CFR 1910.147(d) were not utilized before the employee began the aforementioned work task.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) RC Family Wood Products, LLC on or about January 29, 2016, at the address of 5590 State Route 47, Houston, OH The employer failed to ensure that employees (operators) tasked with service and maintenance activities at or near energized parts of the Woodpecker Nailing Machine were adequately trained in the skills required for the safe application, usage, methods, and means of isolating and controlling hazardous energy sources. Employees tasked with service work on the aforementioned machine were not specifically trained to lockout the machine to protect against amputation injuries prior to performing work at or near nip points and moving parts.
