29 CFR 1926.20(b)(2): Frequent and regular inspections of the job site, materials, and equipment to be conducted by a competent person designated by the employer was not provided by the employer who was responsible to initiate and maintain an accident prevention program:  a- 483 Stanford Hill Road, Block 1159, Lot 19.25 Lakewood NJ: A competent person did not conduct frequent and regular inspections of the jobsite.  Employees on this site were exposed to hazards including, but not limited to fall hazards.  Condition noted on or about 03/13/15.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:  a- 43 Stamford Hill Road, Block 1159, Lot 19.25, Lakewood NJ: Employees wearing full body harnesses without the lanyards attached, were exposed to fall hazards of approximately 22 feet 7 inches to the ground surface below when performing roof shingle installation and other related activities from a roof with a pitch of approximately 6 on 12.  Condition observed on or about 03/12/15.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a- 43 Stamford Hill Road, Block 1159, Lot 19.25, Lakewood NJ: Employees wearing full body harnesses without the lanyards attached, were exposed to fall hazards of approximately 22 feet 7 inches to the ground surface below when performing roof shingle installation and other related activities from a roof with a pitch of approximately 6 on 12. 29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:  a- 43 Stamford Hill Road, Block 1159, Lot 19.25, Lakewood NJ: Employees were exposed to fall hazards, of approximately 22 feet 7 inches to the ground surface below, when utilizing an aluminum extension ladder whose feet were visibly sinking into the ground surface below therefore making the ladder unstable and unlevel.   Condition observed on or about 03/12/15.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  a- 43 Stamford Hill Road, Block 1159, Lot 19.25, Lakewood NJ: Employees were exposed to struck by and fall hazards when utilizing an aluminum extension ladder that contained visibly bent rungs and whose fly section was missing its pull cord with the employees standing on the ladder to extend or collapse the fly section.  Condition observed on or about 03/12/15.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards  a- 43 Stamford Hill Road, Block 1159, Lot 19.25, Lakewood NJ: Employees were exposed to fall hazards, of approximately 22 feet 7 inches to the ground surface below, when utilizing an aluminum extension ladder whose feet were visibly sinking into the ground surface below and that contained missing and damaged components.  These employees had not received ladder hazard recognition training from the company.  Condition observed on or about 03/12/15.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
