29 CFR 1910.107(g)(2):   All spraying areas were not kept free from the accumulation of deposits of combustible residues as practical, with cleaning conducted daily if necessary. Scrapers, spuds, or other such tools used for cleaning purposes shall be of nonsparking material:      (a) Finishing Area:  Employees conducts spray painting of wooden cabinet components inside a ventilated spray booth.  The interior sides of the spray booth were deposited with combustible overspray on the walls, as determined on March 10, 2015.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)  Old Door Shop:  The employer did not ensure that machine guarding was provided on a HDC Tools drill press (serial #1009159, model #FDM-16) to protect employees from the rotating chuck and the unused portion of the bit, as determined on March 10, 2015.

29 CFR 1910.213(h)(3): Radial saw(s) were not provided with an adjustable stop to prevent the forward travel of the blade beyond the position necessary to complete the cut in repetitive operations:   (a)  Rough Mill:  Employees operate a DeWalt 7790 12 inch radial arm saw to cut wooden cabinet components.  The bottom portion of the blade of the radial arm saw could be extended up to 4 inches past the cutting table in forward traveling position, as determined on March 10, 2015.

29 CFR 1910.1048(d)(3)(ii): When the last monitoring results for formaldehyde revealed employee exposure at or above the action level, monitoring of employees was not repeated at least every 6 months:       (a) Finishing Area:  The employer did not conduct employee exposure formaldehyde monitoring at least every six months after monitoring conducted on October 22, 2010 and November 8, 2013 indicated that employee exposure was above the action level, as determined on March 10, 2015. The Mountaineer Cabinet Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1048(d)(3)(ii), which was contained in OSHA inspection number 313371411, citation number 1, item number 7 and was affirmed as a final order on April 22, 2010, with respect to a workplace located at 1917 Locust Avenue, Fairmont, WV  26554.

The employer did not institute a medical surveillance program for employees exposed above the action level of 0.50 ppm, as determined on May 11, 2015. The Mountaineer Cabinet Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1048(l)(1)(i), which was contained in OSHA inspection number 313371411, citation number 1, item number 8 and was affirmed as a final order on April 22, 2010, with respect to a workplace located at 1917 Locust Avenue, Fairmont, WV  26554.
