(a) On or about March 10, 2015 employer did not maintain workplace free from slipping hazards due to a rust prohibiter, trash and water that had accumulated on the floor of the mop storage area and the steel roll storage area.      (b) On or about March 10, 2015 the maintenance shop area was maintained in a clean and sanitary condition. Paper boxes, bags of materials were observed on the floor resulting in poor housekeeping.
