29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent): a. Steel Technologies, LLC - Ottawa, OH: On or about April 2, 2015, employees were exposed to fall hazards of approximately five feet and up to 16 feet when removing bands from coils staged for processing at the Z-Mill. The platform where coils are staged adjacent to the mill's pay-off mandrel was not equipped with guardrails (or equivalent protection) to protect employees from falling to levels below the pay-off mandrel. b. Steel Technologies, LLC - Ottawa, OH: On or about April 2, 2015, employees were exposed to fall hazards of approximately five feet when inspecting steel coils on the left reel of the Z-Mill. The platform that employees access to inspect the coil is not equipped with a standard guardrail that includes a top rail. c. Steel Technologies, LLC - Ottawa, OH: On or about April 2, 2015, employees were exposed to fall hazards of approximately seven feet when accessing the platform adjacent to the T-Mill's pay-off mandrel to put bands on scrap. The platform was not equipped with guardrails (or equivalent protection) to protect employees from falling to the level below the pay-off mandrel. d. Steel Technologies, LLC - Ottawa, OH: On or about March 19, 2015, employees were exposed to fall hazards of approximately seven feet when mopping and/or sweeping the platform adjacent to the T-Mill's pay-off mandrel. The platform was not equipped with guardrails (or equivalent protection) to protect employs from falling to the level below the pay-off mandrel.

Steel Technologies, LLC - Ottawa, OH: On or about April 3, 2015, the employer did not ensure that the energy isolating device that is needed to control energy to the Cincinnati surface grinder machine (s/n 353213 F5V004) was operated so as to isolate energy while employees serviced the machine. The grinder was not locked out when the grinding wheel was changed. b. Steel Technologies, LLC - Ottawa, OH: On or about March 30, 2015, the employer did not ensure that energy isolating devices that are needed to control the energy to the left reel of the T-mill were operated so as to isolate energy while employees set-up the machine. The mill was not locked out when employees installed the 24-inch ID plates to the left reel.

The Z-mill's left reel was not guarded to protect employees who inspected coil on the reel. b. Steel Technologies, LLC - Ottawa, OH: On or about March 15, 2015, the employer did not ensure that employees were protected from pinch points that are created as the tail end of coil is banded on the Z-mill's right reel. Employees are exposed to pinch points created as the coil is released from the reel at a time employees apply the band. c. Steel Technologies, LLC - Ottawa, OH: On or about April 2, 2015, the employer did not ensure that employees were protected from the nip point created where steel passes over the T-mill's rotating deflector roll. d. Steel Technologies, LLC - Ottawa, OH: On or about April 8, 2015, the employer did not ensure that the chuck of the Cincinnati 14x72 surface grinder (s/n 353213 F5V004) was guarded to protect employees from hazards associated with the rotating parts. e. Steel Technologies, LLC - Ottawa, OH: On or about December 11, 2014, employees that used a rag to wipe down the roll stock located in the Cincinnati 14x72 surface grinder (s/n 353213 F5V004) were exposed to the in-going nip point created by the grinding wheel and roll stock.
