29 CFR 1917.112(c)(1): Guardrails were not capable of withstanding a force of at least 200 pounds applied in any direction at mid-span of the top rail, or at the uppermost point if there was no top rail:  Boom access points:  On or about March 12, 2015 and times prior thereto, the thumb latches that held the connection point for the chain were rusted in the open position.  This exposed employees to a potential 80 foot fall to the lower surface

29 CFR 1917.112(c)(3): The top surface of guardrails was not 42 inches, plus or minus 2 inches high:  Boom access:  On or about March 12, 2015 and times prior thereto, the height of the chain top rail measured at 35 inches.  This exposed employees to a potential fall of over 80 feet to the level below.

29 CFR 1917.123(a): Working and walking areas shall be illuminated. Unless conditions described in the regulations of the United States Coast Guard (33 CFR 126.15(1) and (n), and 33 CFR 154.570) exist in the case of specific operations, illumination in active work areas shall be of an average minimum light intensity of 5 foot-candles.   Outside millwright shop:  On or about March 12, 2015 and times prior thereto, the lighting outside the East end of the Millwright building showed 0 foot candles of light.  This caused an employee to get struck-by by a golf cart causing some scraps and bruising.
