29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:           (a)  Geisel Heating and Air Conditioning, Inc. 633 West Broad Street, Elyria, Ohio: On or about March 11, 2015 at the worksite at 120 Filbert Street, Elyria, Ohio, the employee was not trained in electrical safety-related work practices.   While performing troubleshooting activities on a 110 volts AC energized circuit, the employee was exposed to electrical hazards including but not limited to electrical shock.  The employee was not trained in electrical safe work practices and did not use personal protective equipment to protect from the electrical hazards including but not limited to electrical shock.  The employee did not wear insulating gloves to protect from electrical shock.         (b)  Geisel Heating and  Air Conditioning, Inc. 633 West Broad Street, Elyria, Ohio: On or about March 26, 2015 at the worksite at 37800 French Creek Road, Avon, Ohio, the employee was not trained in electrical safety-related work practices.  While performing troubleshooting activities on a 110 volts AC circuit to the dishwasher, the employee was exposed to electrical hazards including but not limited to electrical shock. The employee did not wear insulating gloves to protect from electrical shock.  The employee did not know which personal protective equipment to select and use to protect from the hazards and was at risk of serious injury.         (c)  Geisel Heating and Air Conditioning, Inc. 633 West Broad Street, Elyria, Ohio: On or about March 10, 2015 at the Parker Hannafin worksite, the employee was not trained in electrical safety-related work practices.  While performing troubleshooting activities on a 208 volts 3 phase AC circuit to the rooftop condensing coil, the employee was exposed to electrical hazards including but not limited to electrical shock, arc flash, and arc blast.  The employee did not know which personal protective equipment to select and use to protect from the hazards and was at risk of serious injury.          (d)  Geisel Heating and Air Conditioning, Inc. 633 West Broad Street, Elyria, Ohio: On or about March 26, 2015 at the Market Bar worksite, the employee was not trained in electrical safety-related work practices.  While performing troubleshooting activities on a 208 volts 3 phase AC circuit to the cooler, the employee was exposed to electrical hazards including but not limited to electrical shock, arc flash, and arc blast.  The employee did not know which personal protective equipment to select and use to protect from the hazards and was at risk of serious injury.

An employee, who was not qualified, performed troubleshooting activities on an energized 110 volts AC circuit and did not wear insulating gloves. The employee did not use electrical personal protective equipment while performing the task and was at risk of serious injury for electrical hazards including but not limited to electrical shock.  The employee did not wear insulating gloves while working with or near energized electrical conductors.         (b)  Geisel Heating and Air Conditioning, Inc. 633 West Broad Street, Elyria, Ohio: On or about March 26, 2015 at the worksite at 37800 French Creek Road, Avon, Ohio, the employer did not ensure that an employee working on 110 volts AC circuit to a dishwasher followed safe work practices. The employee performed troubleshooting activities on a 208 volts 3 phase AC circuit to the rooftop condensing coil.  The employee was exposed to electricity and was at risk of serious injury from hazards including but not limited to electrical shock, arc flash, and arc blast.         (d)  Geisel Heating and Air Conditioning, Inc. 633 West Broad Street, Elyria, Ohio: On or about March 26, 2015 at the Market Bar worksite, the employee did not use electrical protective equipment. The employee used a multimeter to perform troubleshooting activities on a 208 volts 3 phase AC energized circuit to the cooler to verify the circuit was.  The employee did not wear arc-rated clothing or insulating gloves and was exposed to electricity.  The employee was at risk of serious injury from electrical hazards including but not limited to electrical shock, arc flash, and arc blast.
