29 CFR 1910.147(c)(1):   The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) A written Hazardous Energy Control Program (Lockout/Tagout) was not developed to ensure employee(s) who perform maintenance and setup of silicone injection molding machines, rubber mills, extruders, grinders, cure ovens, and dryers were not exposed to unexpected energizing, startup or release of stored energy from mechanical equipment, heating equipment and processes associated with production operations.

29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized:     (a) E9 Injection Molding Press: On or about March 23, 2015 a press set-up employee did not employ electrical safety related work practices when conducting continuity tests on 16 amp 480 volt AC heater fuse. Press set-up employees routinely conduct trouble shooting and repair on press electrical heaters to verify electrical fuse continuity.  Employees did not employ electrical safety-related work practices exposing employees to 480 volt AC electrical hazard.    (b) Maintenance employees routinely perform maintenance on electrical equipment, such as but not limited to lighting equipment, electrical disconnects, electrical switches, fuses and electrical control panels. Employees did not employ electrical-safety related work practices when conducting trouble shooting or verification of deenergized electrical circuits exposing employees to electrical hazards up to 480 volts AC.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  (a) Employees in production operations and maintenance are exposed to but not limited to solvents, silicone rubber, oils, and grease. Training was not provided regarding the new label elements (product identifier, signal word, pictogram, hazard statement(s), and precautionary requirements) and new safety data sheet (SDS) format.
