29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasion, and openings through which conductors entered were not effectively closed:    On or about March 17, 2015, in the EPTD Press Area of Building #1, electrical conductors were run through holes in cabinets of machinery which exposed employees to electrical shock hazards in the following instances;      (a)   Bruderer Machinery (straightener), electrical conductors entering the cabinet was not protected from abrasion.      (b)   Bruderer Press #53, electrical conductors entering the cabinet was not protected from abrasion.      (c)   Bruderer Press #55, electrical conductors entering the cabinet was not protected from abrasion.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    On or about March 17, 2015, in the EPTD Press Area of Building #1, electrical cables were not provided with strain relief that would prevent pull from being directly transmitted to joints or terminal screws; employees were exposed to electrical shock hazards in the following instances;      (a)    Next to the OAK LAM Press #30, the strain relief to the 115V WELDOTRON Malfunction Detector Control Box electrical power cable was broken.      (b)   Next to the Minster Press #60, the strain relief to the 115V WELDOTRON Malfunction Detector Control Box electrical power cable was broken.      (c)   Next to the Minster Press #60, the strain relief to the BRUDERER 18260-1000 Sheet & Strip Straightener electrical power cable was broken.
