29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  On March 17, 2015, at a workplace located at 828 Trailwood Drive, Desoto, TX; at least three (3) employees, who used pneumatic nail guns were not protected from eye injuries by wearing protective eyewear.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities six (6) feet or more above lower levels was not protected by guardrail systems, safety net wystems, or personal fall arrest systems; nor did the employer develop and implement a fall protection plan which met the requirements of paragraph (k) of 1926.502:  On March 17, 2015, at a workplace located at 828 Trailwood Drive, Desoto, TX; at least three (3) employees, who were working on the roof of a 2 story residential installing roofing shingles, were not protected from falling more than 18-feet to ground level below.

29 CFR 1926.1053(b)(16):  Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identified them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  On March 17, 2015, at a workplace located at 828 Trailwood Drive, Desoto, TX; at least three (3) employees, who used a 17-ft extension ladder to access/exit the roof of a 2 story residential home to install roofing shingles, were not protected from falling more than 18-feet to ground level below.
