29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) For employees working at 306 65th Street in Holmes Beach, as observed on or about March 17, 2015. Employees were installing plywood sheathing on the roof without the use of a personal fall arrest system. The roof pitch was 6:12 and the eave to ground height measured approximately 14 feet 5 inches.

Employees were observed using a ladder to access the front porch roof to install roof sheathing that did not extend a minimum of 3 feet above the upper landing surface. The top of the ladder was positioned underneath the eave.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder: a) For employees working at 306 65th Street in Holmes Beach, as observed on or about March 17, 2015. Employees were using an extension ladder to access the 6:12 pitch roof that was set up in such a manner where the horizontal distance from the top support to the foot of the ladder exceeded one-quarter of the working length of the ladder. The horizontal distance from the top support to the foot of the ladder was approximately 8 feet 6 3/4 inches and the working length of the ladder was approximately 9 feet.
