29 CFR 1926.451(g)(1): Each employee on a scaffold 10 feet above a lower level was not protected from falling to the lower level: (a) Murray, Daniel dba MDB Builders at 1215 Tamarack Ave., Boulder, CO 80304: On and before March 18, 2015, the employer did not ensure that employees were protected from a fall from elevation in that guardrails or other suitable fall protection were not properly installed on the working platform of the pump jack scaffold. The scaffold lacked mid rails, top rails, and end rails. This condition exposed the employees to a fall of approximately 13 feet.
