29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    a) Rear Ground, Back Yard of Structure -  Workers were forced to walk over scrap lumber, bricks, material, trash and other debris in order to access the work area, on or about 3/19/2015.    Abatement certification is required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Roof Top Front, Rear and Sides of Structure - Four workers  laying sheets of plywood  were not  protected from falling approximately 15' off the sides to the neighboring roof or 35' off the rear and front of the structure to the ground below, on or about 3/19/205.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    a) Rear, Left Side  - A 6' fiberglass step ladder was in the folded, closed position, leaned against the structure and used as a non-self supported, extension, ladder to gain access to the first level on or about 3/19/2015.    Abatement certification is required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      a) Rear Center, Ground Level to 2nd Floor - Ladder used to gain access to the second level from outside, extended approximately 24" past access landing on or about 3/19/2015.     Abatement certification is required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
