29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    a) Extruder machine # 10: An employee was exposed to an amputation hazard from the number 10 plastic extruder machine that did not have a guard designed and constructed to prevent the operator from parts, on or about 3/16/2015, an employee sustained injuries to two fingers on his right hand while operating the number 10 plastic extruder machine.
