29 CFR 1910.27(d)(2)(ii): All landing platforms were not equipped with standard railings and toeboards, so arranged as to give safe access to the ladder; platforms shall be not less than 24 inches in width and 30 inches in length:  a) Boiler silo's fixed ladder: On or about March 18, 2015 and at times prior thereto, the employer did not ensure a small platform just above the catwalk was equipped and used with standard railings and toeboards which exposed employees to a fall of 29-feet to the ground below.  b) Boiler silo's fixed ladder: On or about March 18, 2015 and at times prior thereto, the employer did not ensure a small platform just above the catwalk was at least the minimum width and length which exposed employees to a fall of 29-feet to the ground below.
