29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.       a) At 152 W. Ritner St. Philadelphia, PA 19148 - Employees working within 2 feet of the unprotected edge were exposed to a fall hazard of approximately ten (10) feet without the use of a guardrail system, a safety net system, or personal fall arrest system. Observed on or about 19 March 2015.        1- Stairs on the 1st floor.        2- Stairs on the 2nd floor.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    a) 152 W. Ritner St., Philadelphia, PA - Employees performing stucco preparation work were exposed to fall hazards of approximately twenty (20) feet without the walking working platform of the scaffold being fully planked or decked between the front uprights and guardrail supports. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.     a) 152 W. Ritner St., Philadelphia, PA - Employees were not provided with a safe means of access and/or egress to the tubular welded frame scaffold platform from a lower level, exposing employees to a fall hazard of approximately twenty (20) feet.  Employees were using the side frame and crossbraces of the scaffold to access and/or egress the working levels. Observed on or about 19 March 2015.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:    a) 152 W. Ritner St. Philadelphia, PA 19148 - The employer did not ensure that the stair rails were installed on the open sides of each stairway used by employees to access working levels thus exposing the employees to fall hazards. the stairway consisted of approximately 14 risers high per section. This condition was observed on or about 19 March 2015.        1- Stairs on 1st floor.        2- Stairs on 2nd floor.
