29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Nissei Side - Employees worked near and on a plastic extrusion machine #N5 making adjustments and doing start up where the interlock door on the rear of the machine was open and the machine was still operational thereby exposing employees to a serious hazard, observed on or about 3-19-15.

29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided: a) Nissei Side - An electrical control box for the Center Line Shrink Wrap Conveyor had been damaged but was taped together and still in service in an area of pedestrian traffic and employees who put material on the conveyor to be wrapped, thereby exposing the employees to a serious hazard, observed on or about 3-19-15.
