The scaffold poles were not secured at the bottom to prevent displacement.

The wooden scaffold platforms were overlapped by interweaving between the plank rungs of another platform.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.   a) 61 Denton Way, Dallas, GA: Employees were exposed to lower level fall hazards and struck-by hazards while on and around a pump jack scaffold.  The scaffold platform exceeded 10 feet, and its ends extended more than 18 inches over the supports and were not secured.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) 61 Denton Way, Dallas, GA: Employees were exposed to lower level fall hazards of up to approximately 18 ½ -feet while working from a pump jack scaffold platform, in that; no means of fall protection was in use.   Cruz Santos Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1) which was contained in OSHA inspection number 994880, citation number 1, item number 3 and was affirmed as a final order on 12/22/2014 with respect to a workplace located at 4598 Bogan Meadows Drive, Buford, GA 30252.
