29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:  Around the 52 by 62 foot perimeter of the house, there was no fall protection for employees installing rebar on the roof, exposing employees to the hazard of falling from a height of about 12 feet, 2 inches to the ground below.

29 CFR 1926.703(a)(2): Drawings or plans, including all revisions, for the jack layout, formwork (including shoring equipment), working decks, and scaffolds, were not available at the jobsite:  In the front of the house, CMUs were placed underneath shoring jacks that were used to support the forms and the concrete to be poured in the forms. There were no drawings or plans for the formwork and jacks. Employees were exposed to the hazards of falling about 12 feet, 2 inches with the formwork and engulfment in the wet concrete in the event of jacks support failure.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:  At the rear of the house, the ladder did not extend at least 36 inches above the upper landing as a 14 foot, 8 inch long ladder was used to access a 12 foot, 2 inch high eve on a roof, exposing employees to the hazard of falling from a height of 12 feet, 2 inches to the ground below.
