29 CFR 1910.178(l)(2)(ii):     The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    a) 7411 Industrial Park Way, Work floor - On or about February 25, 2015, employees operating powered industrial trucks had not successfully completed the necessary training and evaluation prior to their use.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed.    a) 7411 Industrial Park Way, Work floor - On or about February 25, 2015, employees were working in the area where temporary wiring was installed without the use of fittings to protect the conductor from being abraded by unprotected openings.
