29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) Through out the facility - On or about March 24, 2015, the employer did not have an energy control program for equipment such as but not limited to the press break, sheer, air compressor, iron worker, welders, CNC router, and when outside contractors perform work.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  (a) South end of shop next to roll-up door - On or about March 24, 2015, employee uses an air hose with a metal nozzle attached containing 95 p.s.i. to remove remaining fluid from manufactured parts. These parts come in different sizes and shapes and some are held by the employee to remove excess fluid.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  (a) West side of building - On or about March 24, 2015, the employer did not have each service, feeder, and branch circuit clearly labeled in electrical panel LP1, DP1 and Service Disconnect. Employees were not clear on what breaker to turn off in the event of an emergency that can prevent electrical shock or emergency shut down of some equipment.
