29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).  Employees engaged in masonry activities on a tubular framed welded scaffold did not have the working platforms fully planked  exposing the employees to fall hazards of approximately 21 feet.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation.  Employees engaged in masonry activities on the third level of a tubular framed welded scaffold that did not have mud sills or base plates to support the scaffold exposed the employees to fall hazards of approximately 21 feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  Employees performing masonry activities were accessing upper levels of a scaffold system by means of the crossbraces.  A ladder was not attached or provided exposing employees to fall hazards of approximately 21 feet.

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).  Employees performing masonry activities from a tubular framed welded scaffold that was located less than three feet from energized power lines that were at 300 volts or less exposing the employees to electrical shock hazards.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  An employee engaged in masonry activities was working below other employees without head protection and was exposed to struck-by hazards from falling objects.  Gaspar Fuentes was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 316044635, citation number 1, item number 1 and was affirmed as a final order on February 2, 2012 with a final abatement date of January 12, 2012, with respect to a workplace located at 2800 Cantey St. Fort Worth, TX 76104.
