29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  Near the center of the project, face shields were not worn when guiding a hose from a concrete pump to fill CMUs with concrete, exposing employees to the hazard of being struck in the face and eyes with concrete under pressure in the event of hose failure.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not being protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  Near the center of the project, there was no fall protection for employees on top of the drainage box to pour concrete into the CMUs, exposing employees to the hazard of falling of heights that varied from about 6 to 6.5 feet into the excavation below.

29 CFR 1926.701(b): Reinforcing steel. All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement:  Near the center of the project, there was no protection on dozens of number 5 rebar that protruded vertically about 6 to 18 inches from CMUs, exposing employees working and walking next to the rebars to the hazard of impalement.
