29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: In front of the house, debris was not kept clear of work areas and passageways as there were boards with protruding nails, empty water bottles and pieces of conduit in the path from the parking area to the house. Employees were exposed to the hazard of falling onto the debris.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway were not at least 18 inches (46 cm) wide: On the side exterior of the house, scaffold platforms were not at least 18 inches wide as four 2x4s were used on their sides to provide a 14 inch wide platform. Employees that used the scaffold to plaster the eve were exposed to the hazard of falling from a height of 2 feet onto the debris below.

29 CFR 1926.451(c)(2)(ii): Unstable objects shall not be used to support scaffolds or platform units: On the front and side exterior of the house, CMUs and a 6 foot long step ladder that leaned against the house were used to support scaffold platforms. Employees that used the scaffold to plaster the eve were exposed to the hazard of falling from a height of 2 feet onto the debris below.
