(a) 1215 Manor Drive, Mechanicsburg Pennsylvania - Employees worked on fabricated frame scaffolding that did not have its guys, ties and braces installed at the closet horizontal member to the 4:1 height, on or about March 27, 2015. Abatement certification required within 10 days after abatement date. 29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: (a) 1215 Manor Drive, Mechanicsburg Pennsylvania - Employees worked on a five-buck high fabricated frame scaffold. A competent person did not adequately inspect this scaffold at the start of the work shift to identify visible defects such as ties-ins were not located per the requirements of the standard, top and mid rails were not installed at the proper heights and base plates and mud sills were not level. All of these defects exposed employees to a scaffold collapse or a hazard of falling to the ground below, on or about March 27, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The endpoints at each upright were more than 48 inches (1.3 m) apart. (a) 1215 Manor Drive, Mechanicsburg Pennsylvania - Employees worked on scaffolding without adequate fall protection. The employer did not ensure that a toprail and midrail were provided when cross bracing did not met the requirement to be a top or mid rail, on or about March 27, 2015.
