29 CFR 1910.303(b)(7)(i) is applicable to employment by 29 CFR 1917.1(a)(2)(iv) that incorporates the General Industry requirement: Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment.  (a) On March 25, 2015 the inspection revealed that employees were potentially exposed to serious injury from electric shock due to openings in the supply power box to crane #3. The power box was located in the SW corner of the dock by crane #3 and there was a missing knockout on the right side and smaller openings on the front right corner of the box.  (b) On March 25, 2015 the inspection revealed that employees were potentially exposed to serious injury from electric shock due to openings in the supply power box to crane #2. The power box was located immediately adjacent to crane #2 and there was a missing knockout on the front of the box.

29 CFR 1910.305(b)(2)(i) is applicable to employment by 29 CFR 1917.1(a)(2)(iv) that incorporates the General Industry requirement: All pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose; and each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  (a) On March 25, 2015 the inspection revealed that the junction box on the east wall of the mince room did not have a cover.   (b) On March 25, 2015 the inspection revealed that employees were potentially exposed to serious injury from electrical shock due to exposed live parts on the electrical outlet on the east wall of the mince room, which did not have a cover.   (c) On March 25, 2015 the inspection revealed that employees were potentially exposed to serious injury from electrical shock due to exposed live parts on the electrical outlet on the SW wall of the mince room, which did not have a cover and was damaged.

29 CFR 1910.333(b)(2) is applicable to employment by 29 CFR 1917.1(a)(2)(iv) that incorporates the General Industry requirement: While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2):  (a) On March 25, 2015 the inspection revealed that employees were potentially exposed to serious injury from electric shock when working on the power supply for equipment used to unload white fish, where the power was turned off but not locked out. Located on the dock by crane #1.

29 CFR 1917.23(b)(1): The employer did not ensure that a properly equipped designated person tested the atmosphere in rooms, buildings, vehicles, railcars, or other spaces that contains or had contained a hazardous atmosphere, prior to employee entry to determine whether a hazardous atmosphere existed.   (a) On March 25, 2015 the inspection revealed that employees were potentially exposed to serious health effects when cleaning the Ryco fish storage tank because the air inside the tank was not tested before employees entered the space. The tank was located on the dock between crane #1 and crane #2 at the time of the inspection.

29 CFR 1917.26(f): A US Coast Guard approved 30 inch life ring, with at least 90 feet of line attached, was not available at readily accessible points at each waterside work area where the employees' work exposed them to the hazard of drowning.    (a) On March 25, 2015 the inspection revealed that the life ring located by crane #3 did not have any line attached.

29 CFR 1917.43(c)(5): Powered industrial trucks were not maintained in safe working order.   (a) On March 25, 2015 the inspection revealed that the Toyota Hyster forklift model #BFGU25 was available for use and had a broken strobe light. The forklift was located in the mince room at the time of the inspection.
