29 CFR 1926.102(a)(1):   Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  Employees were exposed to eye injury from the lack of protective safety glasses while using handheld pneumatic nail guns to affix shingles to the roof of a residential property.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(4)(i):   Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  Employees were not protected from a fall through a skylight while performing roofing operations at a residential property, exposing employees to a fall of at least 8' to the lower level.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Employees were exposed to struck by hazards due to the lack of head protection while working below other employees removing shingles and other debris from the roof.  The employer did not ensure that head protection was worn as necessary.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).  Jose Alvarez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 966265, citation number one, item number one and was affirmed as a final order on 5/8/2014, with respect to a workplace located at 1366 Burnett Drive, Aurora, IL 60502.

Jose Alvarez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 966265, citation number one, item number two and was affirmed as a final order on 5/8/2014, with respect to a workplace located at 1366 Burnett Drive, Aurora, IL 60502.
