29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: Inside a warehouse, open toed sandals were worn instead of safety shoes, exposing employees that used shovels and carried five gallon buckets of sealant to the hazard of being struck by objects.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not being protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On the roof of the warehouse closest to Mamie Street, there was no fall protection along the 32 foot by 74 foot perimeter with a 1.5 foot high parapet, exposing employees painting sealant on the roof to the hazard of falling from a height of about 23 feet to the asphalt below.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: At the backside of the warehouse closest to Mamie Street, an extension ladder, consisting of 2-14 foot long sections, did not extend above the top of the 1.5 foot high parapet on the roof. Employees that used the ladder to access and egress the roof were exposed to the hazard of falling from a height of about 23 feet from the top of the parapet to the asphalt below.
