29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:      a. On or about March 30, 2015, at the catwalk over the machine production line, employees cleaning canisters filters were exposed to an 11 feet 6 inches fall hazard to the ground level, when the platform was not guarded with guardrail system.

On or about March 30, 2015, on the east side of the Plant, the employer did not ensure that exit access was at least 28 inches wide exposing employees to tripping/falling and fire hazard. The exit access was reduced to a 23 inches by a metal frame from a machine.

On or about March 30, 2015, in Silo #2, the employer did not ensure that a plate was being installed to cover wire and wire nuts on motor. The lack of plate fails to prevent any electrical arching which would be a source of ignition exposing employees to electrical and fire hazards.
