At 7167 Old Kings N Road, Jacksonville, FL, on or about July 24, 2023 the employer exposed employees to potential hazardous energies in that employees were required to work on equipment to include but not limited to the American Maplan's Vacuum Chamber which had multiple energy sources such as electric and thermal, without those employees having been provided with written machine specific lock out procedure(s) to utilize to ensure that those energy sources were controlled while the employees were performing maintenance or servicing on the machine.

At 7167 Old Kings N Road, Jacksonville, FL, on or about July 24, 2023: the employer exposed employees to electric shock and thermal burns hazards, in that the employer's Lockout/Tagout Procedures for production line #4 did not specifically outline the rules and steps for the control the hazardous energy for equipment with more than one source of hazardous energy, such as the Krauss Maffei's Extruder, which had also thermal energy. b. At 7167 Old Kings N Road, Jacksonville, FL, on or about July 24, 2023: the employer exposed employees to electric shock and thermal burns hazards, in that the employer's Lockout/Tagout Procedures for production line #4 did not specifically outline the scope, purpose, rules and steps for the control of hazardous energy for equipment with more than one source of hazardous energy, such as the American Maplan's Vacuum Chamber, among other machinery.
