29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: a) T-J Roofing & Sheet Metal LLC on or about March 30, 2015, at the address of 5544 Alexis Rd. Sylvania, OH: The employer failed to ensure the safety monitors only responsibility was the monitoring function. The workers charged with monitoring other employees performing roofing work on a low slope roof, partook in activities such as fastening/installing roofing material. These actions took their attention away from employees working near the roofs perimeter. Employees were exposed to a potential 10 ft. fall to the ground below.
