29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a. T-J Roofing & Sheet Metal LLC at the address of 31 N. Erie St. in Toledo, OH: The employer did not ensure that skylights were equipped with a system to prevent employees from falling through the holes. Employees performing roofing work near the skylights were exposed to an approximate nine foot fall hazard through the skylights.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a. T-J Roofing & Sheet Metal, LLC at the address of 31 N. Erie St. in Toledo, OH: The employer did not ensure employees were protected from falls while performing tasks associated with roofing. Employees performing roofing operations on a commercial roof were exposed to falls of approximately nine feet on the unprotected edges of the roof in that a fall protection system was not being utilized.
